Zapalenie tarczycy Hashimoto

Zmęczenie, senność, ciągłe uczucie zimna, bóle stawów, suchość skóry, opuchnięcia, bóle brzucha, wzdęcia, mdłości i zaparcia, zaburzenie rytmu serca, nietolerancja pokarmowa.
Biegamy od lekarza do lekarza, bez skutku szukamy przyczyn, leczymy poszczególne choroby u gastrologa, kardiologa, alergologa, reumatologa etc., a tymczasem może się okazać, że od lat nasz organizm walczy z chorobą autoimmunologiczną, 
w której tarczyca atakowana jest przez układ immunologiczny. Choroba Hashimoto zwana od nazwiska lekarza, który ją opisał. 
By ją zdiagnozować konieczne są badania hormonów - TSH, fT3, fT4, przeciwciała tarczycy oraz USG tarczycy". 
Lekarz rodzinny zleca najczęściej jedynie badanie poziomu TSH, lub dodatkowo fT3, fT4, ale te 3 parametry nie wystarczają, by w pełni zdiagnozować hashimoto. TSH może być w normie, a pacjent czuje się źle i ma narastające objawy. Dlatego w diagnostyce istotne znaczenie ma dokładnie wykonane USG tarczycy z oceną jej objętości, przepływów i echogeniczności"



U pacjentów z Hashimoto oprócz w/wymienionych objawów często występuje refluks, niedobory pokarmowe, anemia, przepuszczalne jelito, alergie pokarmowe, choroby błon śluzowych jamy ustnej, kandydoza i hipoglikemia, jako dodatek do typowych objawów niedoczynności tarczycy, takich jak przyrost wagi, utrata włosów.
Czynnikami odpowiedzialnymi za powstanie Hashimoto, są m.in. nietolerancje pokarmowe i infekcje.
Chorzy na Hashimoto wymagają bezwzględnej zmiany żywienia i stylu życia. 
Do najbardziej pomocnych należą: usunięcie jodowanej soli z diety, pozbycie się fluoru z wody, redukcja stresu, stabilizacja poziomu cukru poprzez stosowanie właściwej diety polegającej na spożywaniu zdrowych protein i tłuszczów z każdym posiłkiem.

Rekomendowane są pokarmy bogate w składniki pokarmowe, takie jak organiczne mięso i warzywa, a także szczególnie polecane dla tych chorych pokarmy fermentowane, które są dobrym źródłem probiotyków.

Dieta nie powinna zawierać pokarmów przetworzonych, cukru i pokarmów uczulających. Do pokarmów najczęściej nietolerowanych zaliczamy gluten, produkty mleczne, soję, kukurydzę, jajka, zboża, rośliny psiankowate i nasiona. Wykluczenie tych pokarmów z diety wpłynie na eliminację procesu zapalnego w organizmie.
Należą do nich m.in: dieta bezglutenowa, dieta wykluczająca jod, specyficzna dieta węglowodanowa, dieta GAPS, dieta Paleo, dieta Paleo autoimmuno.

Teoria molekularnej mimikry a autoimmunologia

Według teorii molekularnej mimikry bakterie czy inne czynniki przypominają (imitują) komórki naszego organizmu, są to tzw. „antygeny własne”.


O związek z powstaniem Hashimoto podejrzewa się wiele infekcji bakteryjnych - zaliczamy do nich: Helicobacter pylori (bakteria, która jest odpowiedzialna za powstanie choroby wrzodowej), Borrelia burgdorferi (odpowiedzialna za powstanie boreliozy) i Yersinia enterocolitica, przewlekłe infekcje -zapalenie wątroby, opryszczki wywołane przez wirusa.

W czasie infekcji czynnik infekujący jest rozpoznawany, jako substancja obca i atakowany przez układ immunologiczny. Jest to sposób na wyeliminowanie infekcji, ale zdarza się, że układ immunologiczny atakuje proteinę czynnika infekującego, która przypomina komórki własnego organizmu. Może to prowadzić do reakcji krzyżowej i na skutek pomyłkowej identyfikacji zaatakowane są „antygeny własne”, a w konsekwencji zostaje zapoczątkowany proces autoimmunologiczny.

Przykładem jest infekcja Streptococcus pyogenes (paciorkowiec ropny). Jest to bakteria, która powoduje zapalenie gardła u dzieci i nastolatków. Czasami zdarza się, szczególnie w przypadkach nieleczonych antybiotykami, że 2-3 tygodnie po przechorowaniu organizm zaczyna produkować przeciwciała skierowane przeciwko paciorkowcowi.

Niefortunnie dla nas, komponent bakteryjnej ściany komórkowej antygenowo przypomina komórki ludzkich zastawek serca i błony maziowej stawów. W konsekwencji, na skutek pomyłkowej identyfikacji, układ immunologiczny atakuje stawy i zastawki serca. Może to doprowadzić do ciężkich powikłań wymagających przeszczepu zastawek, a nawet do śmierci.


Przeciwciała przeciwko 
Yersinia enterocolitica (wskazujące na ekspozycję) u osób z Hashimoto znajdowane są 14 razy częściej w porównaniu z osobami zdrowymi. Błona komórkowa bakterii Yersinia enterocolitica jest zdolna wiązać TSH! Zakażenie tą bakterią może indukować powstawanie przeciwciał przeciwko czynnikom, które rozpoznają i stymulują receptor TSH, takich jak enzym peroksydaza tarczycowa i tyreoglobulina.

Diagnostyka laboratoryjnaH. pylori można wykryć za pomocą badań krwi i w powietrzu wydychanym, a także poprzez badanie kału. Aby zdiagnozować infekcję Borellia przeprowadzamy badanie krwi, a Yersinia może być wykryta poprzez szczegółowe badanie kału.


Leczenie infekcji

Najnowsze teorie autoimmunologiczne podkreślają, że kiedy antygen (czynnik wyzwalający) jest usunięty, produkcja przeciwciał zostaje wstrzymana i “niewinne” tkanki naszego organizmu nie będą więcej atakowane - w przypadku Hashimoto nie będzie atakowane TPO (peroksydaza tarczycowa).

Wyeliminowanie infekcji, jako czynnika wyzwalającego, powinno doprowadzić do zastopowania reakcji autoimmunologicznej, dlatego też uważa się, że wyleczenie infekcji pomoże wyleczyć Hashimoto.

Leczenie chorób autoimmunologicznych antybiotykami:

Terapia poczwórna: Pepto Bismol + Tetracyklina + Metronidazol +Inhibitor pompy protonowej.

Niektórzy odnoszą się z rezerwą do stosowania antybiotyków. Specjaliści medycyny integracyjnej raportują dobre rezultaty przy stosowaniu substancji naturalnych.

Chorobie Hashimoto zawsze towarzyszy stan zapalny w organizmie. Przewlekły stan zapalny obciąża nadnercza i prowadzi do niedoboru witaminy H - biotyny. Ja miałem okazję poznać kilka osób z Hashimoto i czasami przebywanie z taką osobą jest uciążliwe, niektórym z nich po prostu odwalało w gorszych momentach, potrafili zachowywać się uciążliwie.
